I threw one together for my landscape screen. All I did was cut and paste the skill list, combat moves, hero point/danger pool uses and the rules for stuff like Pressure.
Didn't take long to do and it will have what you need if you do it yourself.
Until the official screen lands on my doorstep, I'll be doing the same whenever I get a game going in person. For now it's just relying on cheatsheets and the like while I run it online.

I am not posting it because it was literally cut and paste so I feel wrong about posting it where anybody could grab it up. For your purposes, you could do the same and paper clip it over your old screen.
